I initially was shocked when I saw this Xray. Can you believe that it was just an incidental finding of an otherwise healthy elderly man?
He has thoracic kyphoscoliosis.
The importance of this condition is it may alter the position of the mediastinum in relation to the anterior chest wall, with the result that abnormalities in the trachea and apex beat location may be mistakenly attributed to heart or pulmonary disease (as seen on Xray).
Having said that, severe kyphoscoliosis may have profound effects on pulmonary function, as the chest deformity reduces the ventilatory capacity of the lungs and increases the work of breathing. Patient may develop hypoxia or heart failure at an early age.
